This is a multi-page Minor Subdivision Checklist used to verify that a subdivision application and plat include all required materials and information before processing. It lists application requirements (completed application form, fees, stamped/dated copies, notifications to adjacent owners, Fire/EMS, VFD contact, and GIS/Google aerials), supporting documentation (deeds, covenants, easements, letters from power and telephone companies, forest conservation materials, Critical Area application items, MPDU/TDR/noncontiguous/sliding-scale information), and detailed plat content (ownership, engineers, purpose statement, scale, north arrow, easements, lot areas and numbering, site statistics and buildable calculations, open space designation, roads and access, street names, setbacks, buffers, and proposed public improvements). It also requires land and environmental data on the plat, such as 2’ contour lines, soils mapping, sewer/water location, stormwater facilities, historic sites, 100-year floodplain, wetlands and stream buffers, steep slopes/erosion hazard areas, woodland protection, hydric soils, and a wetlands jurisdictional determination (and note about Army Corps involvement where applicable).
