This is a vendor application/notice from the Queen Anne’s County Office of Budget & Finance that instructs vendors to submit a completed W-9 and the page’s contact information to the Accounting Division (by mail, fax, or email to kprice@qac.org) before the County can issue payment. It summarizes County requirements: vendors must avoid conflicts of interest (report potential issues to the Ethics Committee) and must comply with laws on employment of aliens, with noncompliance treated as a material breach that can lead to contract termination. The form also includes an affidavit for Local Vendor certification — to qualify for local preference a business must have its principal place of business designated in SDAT as Queen Anne’s County or, for sole proprietors, list Queen Anne’s County as their county of residence on their most recent Maryland tax return — and provides signature lines and fields for vendor name, contact person, phone, and date.
