This is the Queen Anne’s County Environmental Health Division Farmer’s Market Vendor Application and accompanying labeling guidance for 2023. The application requires vendors to submit at least two weeks before participating, provide contact and market details, list foods and sources, indicate if they will sample products, and describe transportation, temperature control, contamination protection, and any cold/hot holding equipment; vendors must sign and may need to provide copies of state licenses or product labels. The form states that sampling raw agricultural products, cottage foods, or non-potentially-hazardous foods processed in a licensed plant requires a Seasonal Farmers Market Sampling License from the county. The document also gives detailed cottage food and state (COMAR 10.15.03.12) labeling requirements — name and street address (no P.O. box), product name, ingredient list in descending order, net weight/volume (both metric and English under COMAR), allergen declarations (lists specific allergens), nutritional info if claims are made, required font size and a specific cottage food disclaimer, and provides sample labels.
