Policy Employees will ensure that the information on file with the CPD is current and correct. B. Procedures 1. Employee Information a. Upon appointment, all employees are required to complete a Personnel Information Form, which includes their current address and a telephone number and will provide all pertinent information, including emergency contact information, to the Lieutenant. b. The Lieutenant will ensure that a roster that contains the name, address, phone number and emergency contact information for each unit employee is maintained and updated at least annually. 2. Changes to Employee Information a. All employees will report any change in name, address or telephone number: (1) as soon as possible, but no later than 30 days after the change; and (2) by submitting an updated copy of the Personnel Information Form to the Lieutenant. b. For name changes, employees must also: (1) forward a copy of their new Social Security card to Town Human Resources; and (2) forward proof of the name change (e.g., marriage certificate, final divorce decree, driver’s license) to Town Human Resources.
