This is the Town of Centreville Public Event Permit Application form for non-Town-managed and Town-managed events. It sets a submission deadline (6 weeks before an event; 8 weeks if alcohol is requested), a $50 user fee, and a required $1,000,000 CSL liability insurance certificate (and product liability if food is sold) that must name the Town and include event details. The form collects event specifics (type, dates/times, expected attendance, location, road closures, food vendors, alcohol, portable toilets, sidewalk impacts, trash/recycling plan, and an emergency/public safety plan) and requires an event map and vendor list where applicable. Additional approvals may be required from Queen Anne’s County Health Department, Queen Anne’s County or Maryland State Liquor Boards, and separate agreements/insurance for County-owned properties (Historic Courthouse/Green); road closures and alcohol sales also require Town Council action. The form notes restrictions on using the Centreville Town Seal, lists Town venue ID codes for insurance, and includes Town-use checkboxes for approval, payment, and required documents; a responsible person must sign the application.
